Renew every worker's health certificate before it lapses
A Saudi health certificate for workers in food and public health establishments is issued digitally on Balady and is valid for one year from issue (MOMAH Health Certificate Requirements Guide, section 3.4). It can be renewed from 60 days before expiry and is cancelled if not renewed within 15 days after expiry.
The Ministry of Municipalities and Housing's Health Certificate Requirements Guide (1446 AH / 2025) applies to workers in food-related and public health activities (section 1.3). Its target groups include food and public health activities, seasonal activities and events, the Ramadan, Hajj, Umrah and visit seasons, home food delivery and delivery through apps, mobile and fixed carts, relaxation and body care centres, and home services of food and public health establishments, as set out in the list of activities and professions (section 2.1). A health certificate must be issued for everyone who works inside a food or public health establishment (section 3.4).
To get a certificate, the worker must be legally permitted to work in Saudi Arabia under the Labor Law and be medically fit according to a medical report from a health provider accredited by the Ministry of Health. Workers in professions on the list must also pass the health education test at an approved test centre. Renewal requires the same work permission and medical fitness report, and the certificate is renewed after the fees are paid (sections 3.1 and 3.2). Applications are made on the Balady platform (section 4).
The health certificate is valid for one year from the time it is issued electronically. It may be renewed 60 days before it expires. If it is not renewed within the allowed period of 15 days after expiry, it is cancelled automatically. The certificate must be the digital one from the Balady platform; printed and paper cards are not allowed (section 3.4).

If the deadline passes
A certificate not renewed within 15 days after expiry is cancelled automatically (section 3.4). Violations of the guide are handled under the municipal penalties regulation and its tables, and under the violations table of the Food Law (section 6.2).

For professions on the list of activities and professions, the health education test must be passed every 3 years. A worker who moves from a food activity to a public health activity, or the other way round, must pass the test for the new activity (section 3.4).
